Affordable Care Act (ACA) Form 1095‑C Delivery and Availability Notice

Paperwork Burden Reduction Act

Castle Senior Living provides this notice to explain how Affordable Care Act (ACA) Form 1095‑C is furnished, in accordance with the Paperwork Burden Reduction Act, enacted on December 23, 2024.

Applicability

This notice applies only to individuals who were eligible for Castle Senior Living’s company‑sponsored ICHRA (Individual Coverage Health Reimbursement Arrangement) medical benefit plan during the 2025 calendar year and forward.

Electronic Furnishing of Form 1095‑C

Under the Paperwork Burden Reduction Act, employers are no longer required to automatically furnish paper copies of ACA Form 1095‑C to covered individuals unless a copy is requested.

Castle Senior Living furnishes Form 1095‑C in electronic (paperless) format.

  • Current and former employees may access their Form 1095‑C through their Paycor Employee profile.

Right to Request a Paper Copy

Individuals have the right to request a paper copy of Form 1095‑C at no cost. Please request a paper copy by contacting Human Resources via email at employmentverifications@castlewi.com.

If a paper copy is requested, Castle Senior Living will furnish the form by January 31 or within 30 days of the date of the request, whichever is later.

Compliance Statement

This notice is provided to inform individuals of their right to request a paper copy of ACA Form 1095‑C as required by the Paperwork Burden Reduction Act. Castle Senior Living will continue to comply with all applicable federal requirements. Additional guidance regarding notice language and delivery standards may be issued by the U.S. Department of the Treasury.
