We would like to announce and celebrate that The Grand Hills Castle had a deficiency free state survey!
This survey was conducted by the Wisconsin Department of Health Services and specifically the Division of Quality Assurance. This division conducts both announced and unannounced surveys of assisted living facilities in Wisconsin to ensure that state licensure/certification and home and community-based services (HCBS) requirements are met.
The surveyor evaluated The Grand Hills Castle’s performance and compliance with applicable laws and standards in the areas of resident rights, program services, nutrition and food service, physical environment and safety, medication, and staff training.
